The project is not part of the patients’ care plan. The iCare4Me Project collaborates with many teams within the Hospital of the University of Pennsylvania (HUP) and Perelman Center for Advanced Medicine (PCAM). Our information was distributed through MyPennMedicine to all patients within the Heart and Vascular Center.

It is a yearlong project consisting of a baseline, 1, 3, 6, 9, and 12-month follow-up. The baseline survey takes approximately an hour and a half. Each additional follow-up can take anywhere from 30 to 60 minutes. Additionally, if you are chosen for the intervention, you will have health coaching for the first 6 months of the program.

No, but the patient is more than welcome to participate. If the patient decides to participate, they will participate in a baseline survey and a 12-month follow-up. The patient will be given a 25-dollar ClinCard for their participation.

No. Caregivers receive a total of 200 dollars for participating in the project.

No, only one caregiver per patient may participate.

After you are enrolled, we are available by phone and email to help with any technology issues that may arise.

No, participation is done completely virtually.
